This file is not editable, but if there is anything you disagree with you are more than welcome to strike through it.

The final page is optional. If you feel it is necessary, it is to be filled out in front of a notary, then signed and stamped by them. Notarization is the official fraud-deterrent process that assures the parties of a transaction that a document is authentic, and can be trusted.
